Valproic acid was first synthesized in 1882 by Beverly S. Burton as an analogue of valeric acid , found naturally in valerian . [66] Valproic acid is a carboxylic acid , a clear liquid at room temperature. For many decades, its only use was in laboratories as a "metabolically inert" solvent for organic compounds. In 1962, the French researcher Pierre Eymard serendipitously discovered the anticonvulsant properties of valproic acid while using it as a vehicle for a number of other compounds that were being screened for antiseizure activity. He found it prevented pentylenetetrazol -induced convulsions in laboratory rats . [67] It was approved as an antiepileptic drug in 1967 in France and has become the most widely prescribed antiepileptic drug worldwide. [68] Valproic acid has also been used for migraine prophylaxis and bipolar disorder. [69]

1) For Patients Not Currently Treated with an erythropoiesis-stimulating agent (ESA):

Initial dose: mcg/kg body weight administered as a single IV or subcutaneous injection once every two weeks

Epoetin beta-methoxy polyethylene glycol should be dosed to achieve and maintain hemoglobin between 10 and 12 g/dL. Once the hemoglobin has been maintained within this range, epoetin beta-methoxy polyethylene glycol may be administered once monthly using a dose that is twice that of the every two week dose and subsequently titrated as necessary.

2) For Patients Currently Treated with an erythropoiesis-stimulating agent (ESA):

Epoetin beta-methoxy polyethylene glycol can be administered once every two weeks or once monthly to patients whose hemoglobin has been stabilized by treatment with an ESA. The dose of epoetin beta-methoxy polyethylene glycol, given as a single IV or subcutaneous injection, should be based on the total weekly ESA dose at the time of conversion.

If the previous weekly epoetin alfa dose was less than 8000 units per week or the previous weekly darbepoetin alfa dose was less than 40 mcg per week, then the epoetin beta-methoxy polyethylene glycol dose would be 120 mcg per month or 60 mcg every two weeks.

If the previous weekly epoetin alfa dose was from 8000 units to 16,000 units per week or the previous weekly darbepoetin alfa dose was from 40 mcg to 80 mcg per week, then the epoetin beta-methoxy polyethylene glycol dose would be 200 mcg per month or 100 mcg every two weeks.

If the previous weekly epoetin alfa dose was greater than 16,000 units per week or the previous weekly darbepoetin alfa dose was greater than 80 mcg per week, then the epoetin beta-methoxy polyethylene glycol dose would be 360 mcg per month or 180 mcg every two weeks.

Cardiovascular disease: This medication often affects blood pressure and can contribute to symptoms of heart disease. If you have high blood pressure, heart disease such as heart failure, angina, or a previous heart attack, discuss with your doctor how this medication may affect your medical condition, how your medical condition may affect the dosing and effectiveness of this medication, and whether any special monitoring is needed. If you are taking blood pressure pills to control blood pressure, it is important to continue taking them regularly while you are on epoetin alfa. Your doctor will monitor your hemoglobin (the protein in red blood cells that carry oxygen) until an appropriate dose of epoetin alfa has been determined.

Dihydrotestosterone (DHT) (referred to as androstanolone or stanolone when used medically) can also be used in place of testosterone as an androgen. The availability of DHT is limited; it is not available in the United States or Canada, for instance, but it is available in certain European countries, including the United Kingdom , France , Spain , Belgium , Italy , and Luxembourg . [5] DHT is available in formulations including topical gel, buccal or sublingual tablets, and as esters in oil for intramuscular injection. [6] Relative to testosterone, and similarly to many synthetic AAS, DHT has the potential advantages of not being locally potentiated in so-called androgenic tissues that express 5α-reductase (as DHT is already 5α-reduced) and of not being aromatized into an estrogen (it is not a substrate for aromatase).
